			<content:encoded><![CDATA[<p>Revision rhinoplasty is one of the most difficult facial reconstructive procedures. Often a secondary, tertiary or quaternary rhinoplasty has already been performed leaving the nasal cartilaginous framework ravaged and the skin overlying it scarred and abnormal.</p>
<p>This is one reason to get your rhinoplasty performed in Sydney by a well qualified rhinoplasty expert. This is either an ENT trained Facial Plastic Surgeon such as Dr Pincock or a Plastic surgeon who specialises in the face.</p>
<p>If you have already had a primary rhinoplasty and are unhappy with the results its always best to discuss this with your original surgeon first. You should not be embarrassed or reluctant to bring up certain issues with your rhinoplasty. The best rhinoplasty surgeon in the world has revision rhinoplasty patients and it is therefore not a reflection on your surgeon if you have an issue BUT how he deals with it.</p>
<p>If you are unhappy with seeing your primary surgeon again or have lost rapport, then seeing another surgeon may be necessary.</p>
<p>Remember although choosing your surgeon wisely for a first time rhinoplasty is important it is imperative that you do your research before seeing someone for a secondary rhinoplasty.</p>
<p>Your secondary rhinoplasty surgeon should not be contradictory to your original surgeon or make disparaging comments about your original surgery. It is impossible for the second surgeon to have an understanding of all the factors that led to the issue or have a complete understanding of the conditions of the original surgery.</p>
<p>Revision surgery may simply involve taking down a dorsal hump down a little further or filing down some unevenness from the osteotomies OR it may require a full reconstructive rhinoplasty requiring ear cartilage grafts, rib or even artificial material.</p>
<p>Remember that the nose is not just for looking good so at any stage of rhinoplasty, maintenance of a good nasal airway should be a primary goal for all rhinoplasty surgeons. If you get a blocked nose from a rhinoplasty it will drive you crazy!!!, luckily nasal obstruction following rhinoplasty or &#8220;post rhinoplasty nasal obstruction&#8221; is relatively easy for an Ear, Nose and Throat surgeon to correct.</p>

			<content:encoded><![CDATA[<p>Blocked nose is a common complaint from patients and septoplasty has been the traditional operation to correct this frustrating medical problem. Research has shown that a deviated septum is the only issue in nasal obstruction in very few cases. More commonly a blocked nose is from a combination of septal deviation, turbinate elargement and mucosal inflammation. A fourth factor that is commonly unrecognised by ENT surgeons is Alar segment collapse. This is where the side walls of the nose are unable to resist the inward pressure created when an inspiration through the nose is initiated.</p>
<p>Dr Pincock has learned of several techniques through his training in the United States to overcome this problem. Four cartilage grafts are harvested from the nasal septal cartilage that would otherwise be thrown away and inserted into key areas of the nasal valve to prevent nasal collapse. This reduces the collapse of the alar side wall and significantly increases nasal airflow</p>

<p>I like to use them in 2 main ways.<br />
1) as an alternative to surgical rhinoplasty for mild dorsal defects and tip asymmetry<br />
2) to temporarily correct a contour issue after surgical rhinoplasty.</p>
<p>In the first instance sometime patients want to mask an issue of he nose such as a small depression or bump. This can be easily achieved with dermal filler and is safe, quick and the result is excellent if the right patient is chosen.</p>
<p>2) after rhinoplasty there may be small asymmetries or contour issues while we are waiting for all of the swelling to settle. This is a perfect situation to use temporary filler to help get through that period.</p>

The international certification process is aimed at setting similar standards of knowledge in the area of facial plastic surgery worldwide. The only established certification in the area of facial plastic surgery is that offered by the American Board of Facial Plastic and Reconstructive Surgery (ABFPRS). Specialists who have trained outside North America are not eligible for ABFPRS certification. Keeping this in mind, and with the support of the ABFPRS and the AAFPRS, a Unified International Certification Program in Facial Plastic Surgery was developed. Eligibility criteria were established following the standards of excellence established by the ABFPRS. Today, if international specialists fulfill the criteria, they may be eligible for the Unified International Certification Program sponsored by the International Federation of Facial Plastic Surgery Societies (IFFPSS) and the national facial plastic surgery society in their country or geographic area.</p>
<p><strong>To achieve international certification the surgeon must:</strong></p>
<p>Have completed an approved residency program in otolaryngology/head-and neck surgery or plastic surgery.</p>
<p>Have earned prior certification by the National Examining Body in Otolaryngology/Head-and Neck Surgery or Plastic Surgery.</p>
<p>Have been in practice a minimum of two years.</p>
<p>Have 100 consecutive operative reports (in the area of facial plastic and reconstructive surgery) performed in a two-year period, accepted by a peer-review committee.</p>
<p>Successfully pass a two-day written and oral examination in the specialty.</p>
<p>Hold the appropriate licensure and adhere to the IFFPSS Code of Ethics.</p>

A short scar face lift or a MACS lift may work perfectly well for a 45 year old female with some early jowling but will not produce a great result in a 65 year old patient with a heavy neck and jowls.</p>

<p>To make things easier a Deep Plane facelift is a more extensive facelift that includes a necklift which is a great procedure for patients with a heavy neck and jowls. All the other techniques are more MINIMAL procedures that are for patients that do not have the same rejuvenation requirements.</p>
<p>Threadlifts were all the rage a few years ago because they promised equal results with minimal downtime &#8211; problem is they DO NOT give you long term results.</p>
<p>Each patient is differnt and the approach needs to be tailored specifically to that patients requirements and goals, the best way is to visit your Facial Plastic Surgeon or Plastic Surgeon with an interest in facial procedures to discuss the possibilities and possible outcomes.</p>

<p><strong>Dr</strong> <strong>Pincock</strong> recently travelled to <strong>Queenstown</strong> in New Zealand to talk to his colleagues about the latest techniques in facial plastic surgery. The meeting was the winter meeting of the Australasian Academy of Facial Plastic Surgery <a href="http://www.aafps.com.au/">www.aafps.com.au</a>, in particular the meeting focused on the latest techniques in rhinoplasty, facelift and necklift, the visiting speaker was Dr Keith La Ferriere a well known and outstanding Facial Plastic Surgeon based in the United States. He provided great insight into more advanced necklift (neck lift) techniques such as the Feldman corset platysmoplasty which Dr Pincock is performing back in Sydney Australia.</p>
