Revision Rhinoplasty in Sydney

Monday, August 16th, 2010

Revision rhinoplasty is one of the most difficult facial reconstructive procedures. Often a secondary, tertiary or quaternary rhinoplasty has already been performed leaving the nasal cartilaginous framework ravaged and the skin overlying it scarred and abnormal.

This is one reason to get your rhinoplasty performed in Sydney by a well qualified rhinoplasty expert. This is either an ENT trained Facial Plastic Surgeon such as Dr Pincock or a Plastic surgeon who specialises in the face.

If you have already had a primary rhinoplasty and are unhappy with the results its always best to discuss this with your original surgeon first. You should not be embarrassed or reluctant to bring up certain issues with your rhinoplasty. The best rhinoplasty surgeon in the world has revision rhinoplasty patients and it is therefore not a reflection on your surgeon if you have an issue BUT how he deals with it.

If you are unhappy with seeing your primary surgeon again or have lost rapport, then seeing another surgeon may be necessary.

Remember although choosing your surgeon wisely for a first time rhinoplasty is important it is imperative that you do your research before seeing someone for a secondary rhinoplasty.

Your secondary rhinoplasty surgeon should not be contradictory to your original surgeon or make disparaging comments about your original surgery. It is impossible for the second surgeon to have an understanding of all the factors that led to the issue or have a complete understanding of the conditions of the original surgery.

Revision surgery may simply involve taking down a dorsal hump down a little further or filing down some unevenness from the osteotomies OR it may require a full reconstructive rhinoplasty requiring ear cartilage grafts, rib or even artificial material.

Remember that the nose is not just for looking good so at any stage of rhinoplasty, maintenance of a good nasal airway should be a primary goal for all rhinoplasty surgeons. If you get a blocked nose from a rhinoplasty it will drive you crazy!!!, luckily nasal obstruction following rhinoplasty or “post rhinoplasty nasal obstruction” is relatively easy for an Ear, Nose and Throat surgeon to correct.

Functional Rhinoplasty and Alar Batten Grafts in Sydney

Monday, July 26th, 2010

Blocked nose is a common complaint from patients and septoplasty has been the traditional operation to correct this frustrating medical problem. Research has shown that a deviated septum is the only issue in nasal obstruction in very few cases. More commonly a blocked nose is from a combination of septal deviation, turbinate elargement and mucosal inflammation. A fourth factor that is commonly unrecognised by ENT surgeons is Alar segment collapse. This is where the side walls of the nose are unable to resist the inward pressure created when an inspiration through the nose is initiated.

Dr Pincock has learned of several techniques through his training in the United States to overcome this problem. Four cartilage grafts are harvested from the nasal septal cartilage that would otherwise be thrown away and inserted into key areas of the nasal valve to prevent nasal collapse. This reduces the collapse of the alar side wall and significantly increases nasal airflow

Fillers and Rhinoplasty in Sydney

Monday, July 5th, 2010

Fillers have definitely made their mark in modern facial aesthetics. But what role if any do they have in rhinoplasty?

I like to use them in 2 main ways.
1) as an alternative to surgical rhinoplasty for mild dorsal defects and tip asymmetry
2) to temporarily correct a contour issue after surgical rhinoplasty.

In the first instance sometime patients want to mask an issue of he nose such as a small depression or bump. This can be easily achieved with dermal filler and is safe, quick and the result is excellent if the right patient is chosen.

2) after rhinoplasty there may be small asymmetries or contour issues while we are waiting for all of the swelling to settle. This is a perfect situation to use temporary filler to help get through that period.

Combination plastic surgery procedures in Sydney

Saturday, July 3rd, 2010

It is common to do multiple facial cosmetic procedures at the same sitting, however one cannot be cavalier about it. Some factors to take into consideration are total anaesthesia time, total surface area of surgical fields, total length of surgery and , surgeon fatigue (both mental and physical), expected time of recovery from the surgery.

Commonly performed multiple facial cosmetic  procedures by Dr Tobias Pincock in Sydney are:
1. rhinoplasty and chin implant
2. rhinoplasty and blepharoplasty
3. facelift and necklift
4. facelift and blepharoplasty
5. facelift and blepharoplasty and browlift
6. rhinoplasty and necklift

If patients desire a body procedure and a facial procedure performed simultaneously then Dr Pincock works very closely with Dr Tavakoli who is amongst Sydney’s best breast augmentation and body cosmetic plastic surgeons. For more info visit www.drtavakoli.com.au

It is important to mention these to your surgeon at the time of consultation as it is impossible to add a procedure on the day of surgery as a “last minute decision”. Many times a patient is afraid to ask because they are worried about what the surgeon will think, but having an excellent result depends on very clear lines of communication between patient and surgeon.

Plan all these procedures carefully with your surgeon and discuss each risk and advantages and disadvantages of combining or separating these procedures.

Although Dr Pincock is not a fan of the “extreme makeover”, when planned carefully multiple procedures can have a dramatic effect on reducing ageing and improving ones appearance and self esteem.

Rhinoplasty in Sydney

Tuesday, February 2nd, 2010

Rhinoplasty is a great procedure, firstly most patients have harboured negative thoughts regarding themselves for long periods of time before they summon the courage to come and talk to me about changing the shape of their nose. Secondly the procedure is extremely successful in the right patient to remove those negative thoughts and increase self confidence.

Rhinoplasty myth (1) – not all patients seeking rhinoplasty get an operation – quite often I will have a heart to heart discussion with a patient about what they want to achieve and whether they actually need a rhinoplasty. I often get patients very fixated on small irregularities on their nose THAT DONT NEED FIXING. There is great beauty in imperfection and it often defines a unique and beautiful face. Think Cindy Crawford, Jennifer Grey, Cameron Diaz just to name a few.

Rhinoplasty myth (2) – it will change the way I look and people will notice. NO – you are still you and you will be surprised by how little people will notice even big changes in nose size and shape.

I perform a huge number of Rhinoplasty surgery procedures in Sydney and am very proud of the results I am achieving in rhinoplasty surgery. For more information on the procedure of rhinoplasty click here:

Internal Rhinoplasty – Is it scarless? or a scam?

Friday, January 29th, 2010

There are 2 main ways to perform rhinoplasty, through an external incision or through internal incisions inside the nose – this is sometimes called scar-less surgery. The reality is that many more manoeuvres can be performed though an external approach rhinoplasty and therefore in most noses a more natural appearance can be achieved.

http://www.sydneyfacialplastics.com.au/cosmetic.html#rhinoplasty

In some cases an internal approach simply cannot achieve the result you are looking for. The “scar” in external rhinoplasty is so small and discreet in almost all people and cannot be seen on close inspection.

So to answer the question is scarless surgery a scam – sort of.  Beware of any surgery that promises “minimal scarring” because it usually means “minimal results”. Any rhinoplasty surgeon should be able to perform internal and external approach rhinoplasty and may have a preference for one over another but should be able to talk about the advantages and disadvantages of both.
